Proof of ID – driver’s license, state photo ID or other government issued identification that includes a photo
Social Security info for your family
Note: If you don’t have a social security number then a Individual Taxpayer Identification Number (ITIN) may be required.
☐ Birthdates for you and family members filing
☐ Banking details – routing and account numbers for direct deposit

☐ Tax Documentation:

Wage and earning statements from all employers for the year (Form W-2, W-2G, 1099-R,1099-Misc)
